Steps to Obtaining Fingerprint Card and Sending to CJIS(Criminal Justice Information Systems) for Out-of-State Applicants

1. Request a Fingerprint Card:

   – Contact the Maryland Board of Nursing (MBON) to request a fingerprint packet via email. Provide your address and reason that you need the packet for an out-of-state application. Email: mbon.backgroundreview@maryland.gov or mailto:mbon.nets@maryland.gov.

2. Receive the Fingerprint Packet:

   – Once you receive the fingerprint packet from MBON, ensure it includes the fingerprint card (FD-258) and any additional instructions.

3. Fill out the front of the fingerprint card with your personal information:

  • Full name
  • Date of birth
  • Place of birth
  • Citizenship
  • Height
  • Weight
  • Race
  • Sex
  • Eye color
  • Hair Color
  • Social security number (if applicable; if not, write “N/A”)
  • Maryland driver’s license number (if applicable; if not, write “N/A”)

  • Do not sign the card until instructed by the person taking your fingerprints. In most cases, this would be the police since you need to get your fingerprints taken at the police station near you.

4. Visit a Local Police Station:

  • Take the fingerprint card and a valid form of identification (e.g., passport) to a local police station or designated fingerprint agency near you.
  • Inform them you need to have your fingerprints taken for a criminal history record check in the United States.

5. Get Fingerprinted:

  • Follow the instructions of the fingerprint technician.
  • Ensure that the fingerprints are clear and complete. The technician should guide you on rolling each finger from one side to the other to capture the full print.
  • Sign the fingerprint card in the presence of the technician if required.

6. Verify and Copy:

  • Before leaving, verify that all required fingerprints are captured without smudges.
  • Make a copy of the front and back of the completed fingerprint card for your records.

7. Pay the fee:
Make a payment for the background check of $31.25 with a check.

Only checks issued by banks in the United States are available, and DaRi does not help you process those checks. You may
1) Get help from an acquaintance in the U.S., receive the check yourself, and send it with a fingerprint card or,
2) enter the United States after receiving your visa and visit the agency to handle it. (Recommended)

8. Mail the Fingerprint Card:

  • Send the completed fingerprint card and check to the address below. The fingerprint card and personal check must be in the same envelop.:

9. Track and Follow-Up:

  • Keep a record of the mailing tracking number to monitor the delivery of your fingerprint card.
  • Check the status of your Criminal History Record Check (CHRC) through the “Look-Up-A-License” feature on the Maryland Board of Nursing’s website.

10. Wait for Results:

  • It may take several weeks for the Board to receive and process the CHRC results.
  • If your name does not appear on the website and it has been at least 6 weeks after mailing the fingerprint card, contact the Maryland Board of Nursing to inquire about the status.
Additional Instructions:

Scan and Save: After completing the fingerprint card, scan the front and back of the card to keep a digital copy for your records.

Submit to MBON: Send a copy of the scanned fingerprint card along with your application to the Maryland Board of Nursing. Ensure that all documents are complete and accurately filled out.

By following these steps, you can ensure that your fingerprinting process is completed correctly and efficiently for your RN application in Maryland.
